You don't have to be in the same group to activate the extract. You just have to both stand in the extract. So a random PMC and a random Scav can both extract together there. You don't group up with them pre game you're supposed to find a random player scav and if you both go to the extract it works. Vice versa if you're playing as scav.

Currently, i think the only way to tell PMCs and Scavs apart are (like you said) just their gear they use, scavs are more likely to have shite weapons and usually lacks armor, helmets or even good looking weapons.
